Purpose: This form is to be used for wells with a capacity greater than 25,000 gallons per day (17 gallons per minute). IF the well is not used for: domestic supply or 10 or fewer households, watering of stock or poultry (except commercial feedlots) and providing water supply for oil and gas explorations. The District reserves the right to request additional information. Submit a separate application for each well prior to drilling for new wells.
